OUTSTANDING DIABETES NURSE EDUCATOR

 

Overview

The Outstanding Diabetes Nurse Educator (ODNE) is given to recognize active members of ADNEP who has gone beyond performing his/her regular duties and responsibilities as a Diabetes Nurse Educator (DNE) in providing service to others; creating a positive impact and change among individuals, families, groups, and communities. The ODNE is one who has made diabetes education and serving as his/her “way of life”.

 

This award will start and will be given in January 2017 and annually during the annual ADNEP General Assembly and Scientific Meeting

 

CRITERIA:

The following shall be the criteria for the outstanding diabetes nurse educator:

1.      Significant contribution as a DNE to

a.      Society (20%)

b.      ADNEP (10%)

2.      Creativity and innovation (10%)

3.      Project/s and activities – sustainability and can be replicated to other settings (10%)

4.      5 C’s of professional caring (50%)

a.      Competence – performance evaluation, CPD units (10%)

b.      Commitment – patriotism, length of service, active participation in community activities (10%)

c.      Compassion – citations, letters from satisfied clients (10%)

d.      Confidence – certification from immediate supervisor (10%)

e.      Conscience – impeccable integrity (10%)

 


GUIDELINES AND PROCEDURES

 

       I.          Search and nomination of candidates


1.      Qualification for nomination


    1.1   A Filipino Registered Nurse with valid PRC license


    1.2   Must be an active member of ADNEP


    1.3   Must be a PNA member of good standing


    1.4   Must be morally upright with no pending legal or ethical case


    1.5   Currently practicing with at least 2 years of experience as a diabetes nurse educator


    1.6   With significant contribution to the promotion of wellness, prevention and management of diabetes and diabetes-related complications through diabetes education


2.      The nomination must be fully endorsed


    2.1   For institutional nomination – the nominee shall be endorsed by the local ADNEP chapter and/or head of the institution


    2.2   For self-nomination – the nominee shall be endorsed by any community leader or church/religious leader/DM client


3.      Nominations should be made on the prescribed ODNE award nomination form


4.      Nomination forms are available at ADNEP National Office and may be downloaded at www.adnep.org


5.      The accomplished nomination form and all supporting documents must be presented or sent via courier service on/or before first week of November of each year to:

6.      Documents of the nominees shall be submitted as soft and hard copies. Hard copy shall be neatly and properly bound and submitted in full with certification or proper authentication signed by a notary public, and properly arranged (tabbed) as follows:


    6.1   Nomination form


   6.2   Executive summary detailing the nominee’s background, scope of activity and exceptional achievements, and how the nominee’s services have been benefited or made a positive impact on individuals, families, groups, and communities and projected a positive image of the nursing professional.


*Text should be typed using a minimum font size of 12, Times New Roman, Calibri, or Arial


    6.3   Personal data sheet


    6.4   Documents and evidences of the following:


        a.     Academic and professional achievement


        b.     Contribution to society


        c.     Contribution to the image of diabetes education, nursing, and/or the profession as a whole


        d.     Creativity and innovation


        e.     Project/s


        f.      5 “Cs” of professional caring

 **Documents and evidences are to be arranged according to the criteria to facilitate evaluation

 

     II.          Preliminary screening for nominee/s


1.  Screening of candidates shall be done by a committee formed by the Board of Directors (BOD) of ADNEP primarily through documentary evaluation. Interviews and actual observation shall be done for further verification, if necessary.


 2.    Qualified candidates must obtain a minimum weighted average of 90%, to be considered as qualified nominee/s, and shall be recommended by the screening committee to the BOD of ADNEP.


 3.    Complete documents of the qualified nominees will be forwarded to the Board of Judges with a recommendation/endorsement letter from the screening committee on/or before the 1st week of December of the current year.

 

    III.          Selection/judging process


 1.      It begins with the review by the Screening Committee of the completeness and authenticity of the documents submitted.


 2.      The Screening Committee shall afterwards turn over the documents of the nominees to the Board of Judges.


 3.      The board of judges shall be composed of:


        a.      ADNEP President


        b.      One ADNEP adviser (i.e. ADNEP past president)


        c.      Chairman of the ODNE Screening Committee (duly designated by the BOD of ADNEP)


        d.      One representative of the clientele (e.g. client, physician, administrator of Institute for Studies on Diabetes Foundation, Inc. [ISDFI])


 4.    The Board of Judges shall individually review and evaluate the documents of nominees during the period from 3rd week of December of the current year. The nominees may be scheduled for an interview as deemed necessary by the Board of Judges.


 5.      The individual results/ratings of the nominees from the Board of Judges shall be placed in sealed brown envelops and forwarded to the Screening Committee Chairman.

    
 6.      The screening committee shall convene immediately to collate and rank the ratings given by the Board of Judges to the nominees (1st week of January)


 7.      To be an awardee, a minimum weighted average of 90% is required; the highest rating nominee shall be awarded as the Outstanding Diabetes Nurse Educator of the Year.


 8.      The decision of the Board of Judges is final and executor, and cannot be overruled by the Board of Directors of ADNEP.


 9.      Results of judging shall be held confidential until the announcement of winner during the General Assembly.

 

   IV.          Notification and announcement


 1.      The awardee shall be formally notified by ADNEP President via fastest reliable means about the result (e.g. e-mail, fax, SMS, or telephone call) at least

             
        two (2) weeks prior to the scheduled award 


        An official written congratulatory communication shall be issued by ADNEP.


 2.      The other nominees shall also be notified officially of the result of the final judging by ADNEP’s awards committee chairperson.

 

     V.          Awarding ceremony and award


 1.      A special ceremony shall be conducted for the ODNE award during the ADNEP General Assembly and Scientific Meeting.


 2.      The awardee shall be provided hotel accommodation and round trip ticket for the general assembly, if necessary.


 3.      He/she shall receive a trophy/plaque and a monetary award, to be determined by the Board of Directors of ADNEP.
